This applies to both athlete and non-athlete (including referees, judges, and time keepers) passbooks. Please bring the following items when registering:

1. Drivers License or Birth Certificate
2. Old passbook (if re-applying)
3. Passport photo (one can be taken at the store if needed)
4. Parent or Guardian if under 18
